Mandatory Forms & Required Action
Manhattan University requires all full-time undergraduate students, degree-seeking international students, resident students, and Division 1 athletic participants to have health insurance.
1. Heatlh Records - Download the required immunization and physical forms by registering at medproctor.com.
Forms are due August 1st. For more information, please visit: manhattan.edu/HealthRecords.
2. Health Insurance - Beginning July 1st, complete a waiver with evidence of health insurance or enroll in the University Student Health Insurance Plan.
Enroll or Waiver is due August 1st.
